Male pattern hair loss (Androgenic Alopecia) is the most common form of hair loss in men. It affects the front, mid-section and the crown area of the scalp (unstable area) Hair loss occurs due to the presence of hormone receptors in the hair follicles in this area.
These receptors are stimulated by the male hormone dihydrotestosterone (DHT), which triggers the hair loss process. There are no such receptors in the so-called Fixed Area, on the back and sides of the head, hence hair in these areas grows throughout life.
This genetic process is inherited and determined by one or both parents.

Type I: Adolescent or juvenile hairline. This hairline usually rests just above the upper forehead crease.  This pattern doesn’t represent balding.

Type II: This type shows progression to the adult hairline. It usually sits about 1.5cm above the upper forehead crease. Some temporal recession may also be seen.   This pattern doesn’t represent balding.

Type III: The earliest stage of male pattern balding. This pattern warrants investigating, and is charactarized by deepening temporal recessions.

Type III vertex: Represents early hair loss at the crown (Vertex) area, with or without the temples.

Type IV: Further frontal hair loss and enlargement of the vertex. Nevertheless, a solid band of hair across the top (Mid-scalp) would still exist, separating the front from the vertex.

Type V: The bald area in the front and in the crown (vertex) continues to enlarge. The bridge of hair separating the two areas (mid-scalp) begins to break down.

Type VI: The connecting bridge of hair at the mid-scalp disappears completely. This leaves a single large bald area on the front and top of the scalp. The hair on the sides and back remains relatively high. (Permanent area)

Type VII:  Extensive hair loss. The hair on the sides and back of the scalp  (permanent area) is a thinner band. (Lower in height than type VI above).

As eyebrows are in a more central position on the face and serve to frame the eyes (the single most important facial element). Unlike the loss of scalp hair, the loss of one’s eyebrows is not viewed as a natural process and is, therefore, not cosmetically acceptable.

Eyebrows may be lost for a variety of reasons including thyroid and other systemic diseases, alopecia areata, burns, tattoos, infections, repeated plucking, congenital inability to grow eyebrows and a genetic tendency for eyebrows to thin, or disappear, over time.

Dear Patient. You will read on many websites that the Strip Technique (FUT) is outdated and leaves a big scar. This is not true. It proves that not many clinics are able to perform it properly. At Dr. Saifi’s clinic we perform both FUT and FUE hair transplantation equally well and with perfect precision. When the patient is a good candidate for both techniques, we leave the decision to patient but of course with our help and guidance.

Strip technique- Follicular Unit Transplantation

Step by step from start to finish:

• Depending on the area to be treated and the number of grafts that need to be transferred, the procedure can take from 4 to 8 hours.
• You will be given a mild sedative and local anesthesia in both the donor and recipient areas.
• There might be some mild discomfort when giving the first one or two local anesthetic injections. Further injections should not be felt as we have devised our unique way of injecting the anesthesia, using ultra-fine needles and injectors. The rest of the procedure should not put you in any discomfort.
• Once the anesthetic has had its full effect, the procedure begins with a thin strip of scalp containing healthy hair follicles being removed with the aid of high magnification.
• With the aid of the stereoscopic dissecting microscopes, the strip is then divided into individual follicular unit grafts (each measuring approximately 1mm in width) – the use of microscopes means that about 30% more hair is harvested than through the use of older techniques.
• While this is happening, small incision sites are made in the recipient area of the scalp using a variety of tiny blades and, when ready, the follicular unit grafts are individually inserted. Again, no discomfort is experienced during this step.
• The grafts are blended in amongst the thinning or balding areas in such a way as to match all the natural growing characteristics of your original hair.
• The site from where the donor strip was sourced is closed using two layers of sutures or stitches. The scar normally heals to form a thin, vague and hard to detect scar amongst your permanent hair.
• We usually use a special type of skin closure knows as Trichophytic Wound Closure. This involves trimming one edge of the incision prior to closing it, so that when the opposing edge of the cut is joined to the trimmed edge, hairs from the trimmed edge grow through the resulting scar, effectively camouflaging its appearance further.
• The entire procedure is very clean and, when completed, no bandages or dressings are usually needed.
• Within half an hour of completion, you can be on your way home. We strongly advise you not to drive if possible. We can happily arrange transportation to and from the clinic if needed.
• Multiple breaks are given throughout the day. Lunch and refreshments are provided, as well as a choice of entertainment material to make the time spent more enjoyable.

FUE (Follicular Unit Excision/Extraction) is currently one of the most popular procedures in the world. The harvesting of grafts, i.e. single hair follicles containing 1 to 4 hairs, eliminates the formation of linear scar, which occurs after the FUT-strip technique hair transplantation procedure. Although a linear scar is not produced with FUE, it does create small scars of hyper-pigmented or hyper-pigmented “dots” which can be visible when the hair is cut very short and mostly in people with darker complexion.

Dr. Saifi uses a special device to make pockets (Sapphire Blades) for the previously harvested hair follicles. He uses a tool which allows to implant the graft in the right direction and at the desired angle. The angle of hair growth and hairstyle design – graft distribution is one of the most important steps to make the hair look natural after the procedure. Dr Saifi, with his 25 years of experience, always personally devotes a lot of time to this step. The many years of experience of the doctor, a well-coordinated and trained team of nurses assisting the doctor. IS THE PROCEDURE PAINFUL?

During the hair transplantation procedure we use local anesthesia and additionally the patient takes a tablet which makes him/her relaxed, thanks to which the discomfort during the hair transplantation procedure is minimal. What is the best tool for FUE? Robot and other new machines for hair transplantation?

Dear patient. A machine is a tool just like a scalpel in the hands of a surgeon. The success of the procedure, natural look and most importantly no damage to the harvested hair during harvesting and implantation is determined only by years of experience of the doctor and the whole team. We use a FUE machine where every move is controlled by a doctor. We do not use a robot because it damages a certain percentage of collected hair follicles which does not occur in our refined method. The treatment time is similar because in Marwan Saifi clinic a large team works on the patient. This eliminates the human fatigue factor during the procedure.

FUE method, when can I expect the results after the procedure and how long do they last?

Usually it takes around 6 months, and the full effect is about 12 months. Hair grows gradually and transplanted hair stays for lifetime, as it is resistant to falling out, because it’s taken from the donor site – an area where the hair is resistant to DHT.

FUE technique- Step by step from start to finish:

FUE is a harvesting technique whereby the individual grafts are punched or ‘drilled’ out from a shaved area of the donor scalp. Although scars are still produced, they usually heal as small dots scattered over a large area. This makes it extremely difficult to notice, even with a short hair cut. It is ideal where the patient prefers a shorter hairstyle, such as a No.1 or No.2 cut. Nevertheless, we recommend discussing this with us in detail during your consultation and following a thorough examination.

•  Currently, we are able to safely transfer up to 3000 follicular unit grafts in a single session (please keep in mind that we are using the word graft not hair, and each graft contains between one and three hairs) This will usually take around 4 to 8 hours (this include short breaks and  lunch break for the patient)
•  If the treatment involves a significant number of grafts, the entire back and sides of the scalp will be shaved on the day of surgery.
•  You will be given a mild sedative and local anaesthesia in both the donor and recipient areas.
•  There might be some mild discomfort when giving the first one or two local anaesthetic injections. Further injections should not be felt as we have devised our unique way of injecting and overlapping the anaesthesia, using ultra fine needles and injectors. The rest of the procedure should not put you in any discomfort.
•  Once the anaesthetic has taken effect, the procedure begins with the mechanically excising single follicular units using 0.7mm – 0.9 mm punches and a variety of other equipment.
•  The end result of this step is multiple small dots on the back and sides, rather than the linear scar of the strip FUT.
•  The entire procedure is very clean and, when complete, no bandages or dressings are usually requires.
•  The transection rate at our practice is usually less than 2% of all attempts.
•   FUE grafts -by nature- can also be more vulnerable due to the fact that there tends to be less protective tissue around each excised graft, but at our practice we make sure that these grafts are hydrated and well handled. The procedure entails extra care in the initial post-operative period to ensure maximal graft survival.
